Dear friends and patients,

I am sorry to tell you I have decided to retire and that my endocrinology practice will close permanently after Friday, February 28, 2020. I will not be able to continue your care or renew your prescriptions after that date.

I love my practice and I care a great deal about my wonderful patients, but it is time for me to move on. I thank my patients and the community for their support over the years. You have placed your trust in me, and every day I have strived to be worthy of that trust, and to deliver high quality, personalized care. I thank my wonderful staff for all their hard work, enthusiasm, and for sharing a belief in treating our patients as well as we can.

All appointments scheduled after Friday, February 21, 2020 are hereby cancelled. I encourage all those with ongoing Endocrinology issues to get appointments with another endocrinologist.

Once you have decided who you will see in the future for your endocrinology needs, you may sign a record release and we will send a copy of your records directly to that doctor. If you want a copy of your records for yourself, you will need to sign a release authorization form and records will be copied for you for a customary and reasonable fee to cover our costs. These copies may be picked up at our office until February 28, 2020. (Depending on your record needs, it may be less expensive to obtain your records from the future records custodian, Desert River Solutions than from our office. They are often able to copy and provide the records at a lower cost than our office can.)

Please see the instructions included with this letter about how to obtain or send copies of your medical records.

It has been my pleasure and my honor to attend to your Endocrinology needs over the years, and I wish you the best of health in the future.
